Another common trap is treating the comparison as optional without justification. You can drop it, but you need to say why. If you're studying something that already has established alternatives, skipping the comparison makes the question weaker, not stronger. Reviewers will ask about it, and you'll need an answer that isn't "I didn't think about it." There's also a practical limitation to keep in mind. PICO works well for therapeutic questions but it's awkward for descriptive or exploratory research. Forcing PICO onto qualitative questions usually produces something that looks like a question but doesn't actually guide the research design. If you're building a search strategy around a PICO question, the next step is translating each component into database terminology. Population becomes MeSH terms and keywords. Intervention gets its own set. Comparison and outcome follow the same pattern. You combine them with AND and OR operators. A well-built search string from a good PICO question usually returns between two hundred and two thousand results in PubMed, depending on how narrow or broad your components are.
