Fordyce granules are enlarged sebaceous glands that appear as small, pale yellow or white bumps. They show up most often on the lips, inside the cheeks, and sometimes on the genital area. They are present in roughly 70 to 90 percent of adults. They are not an infection. They are not contagious. They do not turn into anything worse. The only reason people want to get rid of them is that they bother them visually.
I used to have a client who came in almost yearly asking about removal after seeing someone online claim these spots were "toxic buildup." The truth is boring. These are just oil glands that happened to sit closer to the surface than usual in certain areas. They do not collect toxins. They do not need "detoxing." This misconception alone is responsible for a lot of people buying random creams that do absolutely nothing.
How To Get Rid Of Fordyce Granules Through Medical Procedures
There is no proven home remedy. There is no ointment you can buy over the counter that dissolves them safely. The methods that actually work are clinical procedures performed by a dermatologist or qualified practitioner.
Laser therapy is one of the more common approaches. Pulsed dye lasers or CO2 lasers target the gland tissue and break it down. The area numbed beforehand, and the procedure itself takes maybe 20 to 30 minutes for the lip area. Downtime is usually a few days of redness and crusting. Results tend to be noticeable after one to three sessions, though complete clearance is not guaranteed. Scarring is rare when done by an experienced hand, but it is possible. I once had a patient who went to a budget clinic and came back with a slightly pitted texture on the upper lip where the laser was applied too aggressively. Good providers know their way around facial anatomy; cheap ones do not.
Microneedling is another option, sometimes combined with radiofrequency energy. The idea is to create micro-injuries that stimulate the skin to remodel and reduce the visibility of the bumps. It works better for superficial granules and usually requires multiple sessions spaced a few weeks apart. It is less aggressive than laser but also less effective for deeper or more prominent lesions.
Electrodesiccation uses a fine needle tip to apply electrical current and destroy the gland tissue. It is quick, cheap, and effective in the right hands, but it leaves tiny scabs that take a week or so to fall off. There is a small risk of post-inflammatory hyperpigmentation, especially on darker skin tones.
Excision is basically cutting them out. This is rarely done individually because the granules are numerous and scattered. It might be considered if there is one particularly large or bothersome lesion, but it is not a practical approach for widespread Fordyce granules.
Why Most Home Approaches Fail
People try everything. They squeeze the bumps, which does nothing because these are not whiteheads and there is no pore opening to express. They apply tea tree oil, apple cider vinegar, or toothpaste. None of these remove the glands. Tea tree oil in high concentrations can cause contact dermatitis, which makes the area red and irritated and actually draws more attention to the bumps. Apple cider vinegar is acidic and can burn the delicate skin of the lips. Toothpaste dries the skin out temporarily but does not penetrate to the sebaceous gland.
I had someone tell me they spent three months rubbing garlic on their lips based on a Reddit thread. They ended up with a chemical burn that took another month to heal. The granules were still there afterward. That is not an unusual outcome for people trying DIY methods.
Salicylic acid peels available at home are another common attempt. These are very low concentration compared to what a dermatologist uses in-office. At home strength, they might slightly exfoliate the surface skin but will not reach the sebaceous gland deep enough to make any real change.
The Realistic Outlook
If you go to a dermatologist, expect a consultation where they confirm the diagnosis first. Sometimes people mistake milia, herpes simplex lesions, or other conditions for Fordyce granules. A proper diagnosis matters before any treatment.
Treatment is purely cosmetic. Insurance typically does not cover it. Expect to pay anywhere from a few hundred to over a thousand dollars depending on the method and the area being treated. Laser treatment for the lips is on the higher end.
Results are never permanent in the sense that new granules can become more visible over time, especially during hormonal shifts. Puberty, pregnancy, and certain medications can make them more noticeable. So even after successful treatment, some regrowth or reappearance is normal.
The simplest workaround most people end up accepting is just leaving them alone. They fade somewhat with age as skin thins and changes. They are invisible to everyone except you if you are looking for them, which is most of the time. I have noticed that after spending years trying to treat them, my own visibility anxiety around them decreased significantly. Not because they disappeared, but because I stopped checking them in the mirror every day. That is perhaps the most practical advice I can offer.
