The problem is what blocks the path to that chest. The route requires you to pass through a room where two specific enemies must be defeated in a particular order, or the floor layout shifts and the chest room becomes unreachable until you leave and re-enter the dungeon. I ran into this exact situation during my first playthrough. I cleared the room, grabbed the chest, and the game didn't register the item. I checked the party inventory three times. Nothing. It turned out I had skipped a second encounter on the same floor — one enemy in the eastern wing of the cave that spawns independently of the main path. That enemy has to die too. It does not go in your equipment slot like a normal accessory. You hold it in your inventory and use it from there when the timing is right. The item's function activates when you are at a specific gate later in the game — the gate that opens only when the ring is in your possession and you choose to use it at the correct moment. Using it too early wastes it. Using it too late means you have already triggered another ending condition. There is no way to retrieve it once used, so keep it unequipped and out of the way until you reach the intended gate. A few other things that matter in practice. Save before entering the Dead Moon Cave. The dungeon has no checkpoints. If you make a mistake on the puzzles and a character falls, you are not reloading from mid-dungeon.
