Henry, 2002-08-31 Implantable defibrillators as originally conceived by Michel Mirowski were limited to the detection and automatic termination of ventricular fibrillation. In the original AID device, the detection algoritlun sought to distinguish sinus rhytlun from ventricular fibrillation by identifying the more sinusoidal waveform of ventricular fibrillation. The therapeutic intervention was elicited only once deadly polymorphic rhythms had developed. It was rapidly learned, however, that ventricular fibrillation is usually preceded by ventricular tachycardia. Mirowski recognized the pivotal importance of developing algoritllms based on heart rate. Ventricular tachycardia detection allowed the successful development of interventions for the termination of ventricular tachyarrhythmias before they degenerated into ventricular fibrillation. Current device therapy no longer confines itself to tlle termination of chaotic rhythms but seeks to prevent them. Diagnostic algorithms moved upward along the chain of events leading to catastrophic rhytlulls. Rate smoothing algorithms were developed to prevent postextrasystolic pauses from triggering ventricular and atrial tachyarrhytlmlias. Beyond the renaissance of ectopy-centered strategies, long-term prevention received increasing attention. Multisite pacing therapies provided by Arrhythmia Management Devices were designed to reduce the arrhytlunia burden and optimize the synergy of cardiac contraction and relaxation. Clinical evidence now suggests that atrial fibrillation prevention by pacing is feasible and tllat biventricular pacing may be of benefit in selected patients with heart failure. However, these applications of device therapy that generally require ventricular defibrillation backup remain investigational and were not considered in this book. |

Cassells, 2013-06-29 This book is based mainly on invited and offered papers presented at the Second International Symposium on Bacterial and Bacteria-like Contaminants of Plant Tissue Cultures held at University College, Cork, Ireland in September 1996, with additional invited papers. The First International Symposium on Bacterial and Bacteria-like Contaminants of Plant Tissue Cultures was held at the same venue in 1987 and was published as Acta Horticulturae volume 225, 1988. In the intervening years there have been considerable advances in both plant disease diagnostics and in the development of structured approaches to the management of disease and microbial contamination in micropropagation. These approaches have centred on attempts to separate, spatially, the problems of disease transmission and laboratory contamination. Disease-control is best achieved by establishing pathogen-free cultures while laboratory contamination is based on subsequent good working practice. Control of losses due to pathogens and microbial contamination in vitro addresses, arguably, the most importance causes of losses in the industry; nevertheless, losses at and post establishment can also be considerable due to poor quality microplants or micro-shoots. In this symposium, a holistic approach to pathogen and microbial contamination control is evident with the recognition that micropropagators must address pathogen and microbial contamination in vitro, and diseases and microplant failure at establishment. There is increasing interest in establishing beneficial bacterial and mycorrhizal association with microplants in vitro and in vivo. |

However, initially their beneficial effect in disseminated cancer was of such short duration that the inevitable death of the patient from his primary disease precluded any clinical manifestation of the carcinogenic potential. During the last decade, chemotherapy has radically changed the outlook for many patients with cancer. Combinations of drugs, administered as the primary treatment, have resulted in high rates of cure in patients with disseminated malignancies, such as stage IV Hodgkin's disease or childhood acute lymphocytic leukemia. In other disseminated forms of neoplasia, induction of a remission, a substantial palliation and a prolongation of survival have been achieved. In many instances of localised disease, where surgery with or without radiotherapy are the primary form of treatment, anticancer drugs have been used with success as adjuvant therapy for distant microscopic disease. With these spectacular achievements, secondary malignancies, in particular acute non-lymphocytic leukemia (ANLL), has become of major concern. Incidence Acute leukemia is the most frequent form of secondary neoplasia in patients treated for cancer (Penn 1981). In one large series, 5. 9% of all ANLL could be attributed to previous chemotherapy (Kapadia et al. 1980). |
