can pa's open their own practice: False Premise, False Promise Sally C. Pipes, 2020-01-07 American health care is at a crossroads. Health spending reached $3.5 trillion in 2017. Yet more than 27 million people remain uninsured. And it's unclear if all that spending is buying higher-quality care. Patients, doctors, insurers, and the government acknowledge that the healthcare status quo is unsustainable. America's last attempt at health reform -- Obamacare -- didn't work. Nearly a decade after its passage in 2010, Democrats are calling for a government takeover of the nation's healthcare system -- Medicare for All. The idea's supporters assert that health care is a right. They promise generous, universal, high-quality care to all Americans, with no referrals, copays, deductibles, or coinsurance. With a sales pitch like that, it's no wonder that seven in ten people now support Medicare for All. Doctors, especially young ones, are coming around to the idea of single-payer, too. Democrats, led by the progressive wing of the party, hope to capitalize on this enthusiasm. In 2017, they introduced companion legislation in the House and Senate that would establish Medicare for All. They have already promised to do the same when the next Congress convenes in 2019. More than 70 House Democrats have joined a new Medicare for All Caucus. Senator Bernie Sanders is effectively already on the presidential campaign trail, making his case for single-payer. If Democrats take the White House and Senate in 2020, and hold onto the House, a Medicare for All bill could be among the first pieces of legislation presented to the new president for a signature. In this book, Sally C. Pipes, a Canadian native, will make the case against Medicare for All. She'll explain why health care is not a right -- and how progressives pressing for single-payer are making a litany of promises they can't possibly keep. Evidence from government-run systems in Canada, the United Kingdom, and other developed countries proves that single-payer forces patients to withstand long waits for poor care at high cost. First, she'll unpack the Medicare for All plans under consideration in Congress. She'll explain how radical they truly are. Medicare for All will not save $5 trillion, as some of its proponents claim. It will cost about $32 trillion over 10 years, according to analyses from the Urban Institute and the Mercatus Center. It will outlaw private health insurance. It will raise taxes by trillions of dollars. It will cut pay for doctors to the rates paid by Medicare and thereby exacerbate our nation's shortage of physicians. And it will ration care. Then, Sally will detail the horrors of single-payer. She'll start in Canada, whose single-payer system most closely resembles the one progressives have in mind for the United States. Analyses of the government-run systems in the United Kingdom and a few other developed countries will follow, with particular focus on the problems that these systems pose for patients and doctors. To substantiate her indictment of single-payer, Sally will marshal both quantitative and qualitative evidence. She'll highlight how Americans fare better than their peers in Canada and the United Kingdom on the health outcomes that are directly linked to the quality of a healthcare system, including survival rates for patients with cancer and cardiovascular issues. She'll also explain why the health outcomes where the United States performs poorly relative to other nations, like infant mortality and life expectancy, tell us little about our healthcare system. Sally will pepper her text with heart-wrenching stories of the human costs of single-payer -- of people who were injured, were forced to remain in pain, or even died because their government-run healthcare system delayed or denied care. Too often, evangelists for free markets limit their arguments to facts and statistics -- and fail to appeal to the public's emotions. Sally will feature the stories of individuals and families who have been victims of single-payer systems. These vignettes will help drive home the truth about single-payer -- and why it must not come to the United States. She'll conclude with her vision for delivering the affordable, accessible, quality care the American people are looking for. |

Integrative Learning explores this issue, and considers how intentional learning helps students become integrative thinkers who can see connections in seemingly disparate information, and draw on a wide range of knowledge to make decisions. Written by international contributors who engaged reflectively with their teaching and their students’ learning, the book seeks to develop a shared language of integrative learning, encouraging students to adapt skills learned in one situation to problems encountered in another, and make autonomous connections across courses, between experiences, and throughout their lives. More informed teachers can help students develop the necessary attributes for intentional learning, which include having a sense of purpose, fitting fragmentary information into a ‘learning framework’, understanding something of their own learning processes, asking probing questions, reflecting on their own choices, and knowing when to ask for help. Integrative Learning draws on international research and vast studies to provide the reader with the resources to ensure access to a unified learning experience. The book discusses conceptual and technical tools necessary for facilitating integrative learning across a range of disciplines as well as providing learning pedagogies and considers integrative learning in the context of the relevance of higher education in the complexity and uncertainty of the 21st century. It will appeal to academics and researchers in the field of higher education, as well as those generating higher education curriculums. |

Its aim is to encourage governments to govern fundamentally better in terms of policy and administration. A better understanding of context and identities, imaginization, epistemic pluralism, anti-administration, and the context of economics are examples of what is critical for high effectiveness. The pieces included in this book have been handpicked from the vast academic collection that David Farmer has authored over the last thirty years and which were published in the Journal of Administrative Theory and Praxis and the Journal of Public Administration Education. Collectively, these chapters are intended to help governments use post-traditional public administration theory in order to achieve better praxis. |

can pa's open their own practice: Imposter Doctors Rebekah Bernard, 2023-06-01 When you experience a medical emergency, you expect to be treated by a licensed physician with expertise in your condition. What happens when you look up from your hospital gurney to find that the doctor has been replaced by a non-physician practitioner with just a small fraction of the training and experience? From the co-author of Patients at Risk: The Rise of the Nurse Practitioner and Physician Assistant in Healthcare, the first book to warn of the systematic replacement of physicians, comes Imposter Doctors, an even more frightening exposé of patient endangerment at the hands of for-profit corporate entities and healthcare conglomerates. In the two years since Patients at Risk debuted, the employment of non-physician practitioners has continued to skyrocket. While advocates insist that nurse practitioners and physician assistants are ‘just as good’ as physicians, they are wrong. Despite over fifty years of scientific analysis, there is no conclusive evidence that non-physicians can provide safe and effective medical care without physician oversight. In fact, recent studies have shown the opposite: that the replacement of physicians puts patients at risk. The only cure for today’s healthcare crisis is for patients to become informed about who is providing their care. We must all know the difference in clinician education and training, and demand answers from those who would deprive us of physician-led care. REVIEWS and WORDS OF PRAISE This book is well-written, richly researched, and scientifically based. Imposter Doctors explains how scope expansion has been facilitated by the corporatization of American medicine, and exposes the fallacy of NP/PA and physician equivalency. It is a must-read for anyone concerned about our nation’s healthcare system. --Susan Rudd Bailey, MD, Past President American Medical Association Another frank and hard-hitting discussion from the author of Patients at Risk. While some will likely dismiss this book as aiming to protect the status quo in healthcare, I sincerely hope it creates important conversations about training, qualifications transparency, and public safety. --L Allen Dobson Jr, MD, FAAFP, Editor-in-Chief Medical Economics This follow-up book to Patients at Risk articulates the desperate need for reform to the healthcare system to re-insert physicians as the ultimate decision maker for the sake of patient care. After reading this book, one must ask will a physician be available to care for me and my family when the need arises? --Linda Lambert, FAAMSE |

can pa's open their own practice: Reflective Practice in Teaching Gretchen Geng, Pamela Smith, Paul Black, Yoshi Budd, Leigh Disney, 2019-07-17 This book investigates the ways in which pre-service teachers develop and articulate their professional knowledge by presenting their reflections on contemporary issues and topics they have explored during their own teaching practicums. It uses reflective practice to connect pre-service teachers’ personal backgrounds with their placement experience concerning a self-selected topic, including teacher educators’ reflections on the pre-service teachers’ reports on these placement topics. By illustrating the broad range of issues encountered by pre-service teachers, sharing multiple perspectives on the complexity of classroom practice, and demonstrating the importance of reflective practice, it also provides a valuable mentoring framework. Moreover, the book studies how examining pre-service teachers’ life experience can facilitate in-depth understanding, specifically in the context of pre-service teachers’ reflections on their own practices in different educational settings. In short, the book helps current and prospective pre-service teachers and teacher educators get to know their students and themselves better using reflective practice. |

At more than 3 million in number, nurses make up the single largest segment of the health care work force. They also spend the greatest amount of time in delivering patient care as a profession. Nurses therefore have valuable insights and unique abilities to contribute as partners with other health care professionals in improving the quality and safety of care as envisioned in the Affordable Care Act (ACA) enacted this year. Nurses should be fully engaged with other health professionals and assume leadership roles in redesigning care in the United States. To ensure its members are well-prepared, the profession should institute residency training for nurses, increase the percentage of nurses who attain a bachelor's degree to 80 percent by 2020, and double the number who pursue doctorates. Furthermore, regulatory and institutional obstacles-including limits on nurses' scope of practice-should be removed so that the health system can reap the full benefit of nurses' training, skills, and knowledge in patient care. In this book, the Institute of Medicine makes recommendations for an action-oriented blueprint for the future of nursing. |
