Just as the major causes of morbidity and mortality have changed, so too has the understanding of health and what makes people healthy or ill. Research has documented the importance of the social determinants of health (for example, socioeconomic status and education) that affect health directly as well as through their impact on other health determinants such as risk factors. Targeting interventions toward the conditions associated with today's challenges to living a healthy life requires an increased emphasis on the factors that affect the current cause of morbidity and mortality, factors such as the social determinants of health. Many community-based prevention interventions target such conditions. Community-based prevention interventions offer three distinct strengths. First, because the intervention is implemented population-wide it is inclusive and not dependent on access to a health care system. Second, by directing strategies at an entire population an intervention can reach individuals at all levels of risk. And finally, some lifestyle and behavioral risk factors are shaped by conditions not under an individual's control. For example, encouraging an individual to eat healthy food when none is accessible undermines the potential for successful behavioral change. Community-based prevention interventions can be designed to affect environmental and social conditions that are out of the reach of clinical services. Four foundations - the California Endowment, the de Beaumont Foundation, the W.K. Kellogg Foundation, and the Robert Wood Johnson Foundation - asked the Institute of Medicine to convene an expert committee to develop a framework for assessing the value of community-based, non-clinical prevention policies and wellness strategies, especially those targeting the prevention of long-term, chronic diseases. At the request of the National Institute on Aging of the U.S. Department of Health and Human Services, Reducing the Impact of Dementia in America assesses the contributions of research in the social and behavioral sciences and identifies a research agenda for the coming decade. This report offers a blueprint for the next decade of behavioral and social science research to reduce the negative impact of dementia for America's diverse population. Reducing the Impact of Dementia in America calls for research that addresses the causes and solutions for disparities in both developing dementia and receiving adequate treatment and support. It calls for research that sets goals meaningful not just for scientists but for people living with dementia and those who support them as well. By 2030, an estimated 8.5 million Americans will have Alzheimer's disease and many more will have other forms of dementia. Focusing on the parish of St. Sabina's in the southwest Chicago neighborhood of Auburn-Gresham, Eileen McMahon takes a penetrating look at the response of Catholic ethnics to life in twentieth-century America. She reveals the role the parish church played in achieving a cohesive and vital ethnic neighborhood and shows how ethno-religious distinctions gave way to racial differences as a central point of identity and conflict. For most of this century the parish served as an important mechanism for helping Irish Catholics cope with a dominant Protestant-American culture. Anti-Catholicism in the society at large contributed to dependency on parishes and to a desire for separateness from the American mainstream. As much as Catholics may have wanted to insulate themselves in their parish communities, however, Chicago demographics and the fluid nature of the larger society made this ultimately impossible. Despite efforts at integration attempted by St. Sabina's liberal clergy, white parishioners viewed black migration into their neighborhood as a threat to their way of life and resisted it even as they relocated to the suburbs. The transition from white to black neighborhoods and parishes is a major theme of twentieth-century urban history. The experience of St. Sabina's, which changed from a predominantly Irish parish to a vibrant African-American Catholic community, provides insights into this social trend and suggests how the interplay between faith and ethnicity contributes to a resistance to change.
